Breast Reconstruction with the DIEP Flap (Deep Inferior Epigastric Perforator)

 

Breast reconstruction using the DIEP flap represents the most advanced technique for restoring a natural breast after mastectomy. The DIEP flap breast reconstruction is an advanced surgical procedure that utilises the adipose and skin tissue from the lower abdomen to reconstruct a natural breast following mastectomy. This post-oncological micro-surgery technique allows for the achievement of high-level aesthetic and functional results.

 

The DIEP flap involves harvesting a section of skin, adipose tissue, and blood vessels from the lower abdomen without damaging the abdominal muscles. The harvested tissue is then transferred to the breast through microanastomoses, reconnecting the blood vessels of the flap to those in the chest.

 

This surgical technique offers numerous advantages over other breast reconstruction methods, such as prosthetic reconstruction. Firstly, the transferred tissue has a texture and sensation similar to that of natural breast tissue, providing highly satisfactory aesthetic results. Moreover, there is no risk of tissue rejection, as the tissue is taken from the same patient. Unlike the Transverse Abdominis Myocutaneous (TRAM) flap, the DIEP flap does not require the removal of abdominal muscles, thereby reducing the risk of post-operative complications and allowing for a quicker recovery.

 

The ideal candidates for reconstruction with the DIEP flap are those with a sufficient amount of skin and adipose tissue in the abdomen. This technique is particularly suitable for patients who have undergone radiotherapy, as it minimises the risk of complications. However, not all patients are candidates for this procedure; for example, those who have previously undergone an abdominoplasty or complex laparotomy may not be suitable.

 

The duration of the operation varies from 5 to 7 hours, depending on the patient’s characteristics. Recovery generally takes about 4 to 6 weeks, during which pain may be present, and some drains may be placed to prevent the formation of haematomas.
